Authorised payment institutions and authorised e-money institutions, like most FCA firms, submit an annual controllers report within four months of their accounting reference date (SUP 16.4.5R). With a 31 March year end, it's due by 31 July.

What it covers
The identity of everyone who controls your firm, so the FCA can check you can still be supervised effectively. It's submitted electronically on the form in SUP 16 Annex 37A.
It doesn't replace change in control notices
The annual report is a yearly snapshot. Changes in control still have to be notified separately when they happen, under SUP 11 and the rules that apply to your type of firm. Keep track of who your controllers are during the year (SUP 11.4.10R).
Groups
A group can submit one report for all its firms, as long as it meets each firm's due date and lists every firm's reference number (SUP 16.4.7G).
